How to Report
If you believe you have experienced an adverse event related to a Hizashi product, please contact us with the following information:
- Your name and contact information
- The product name and where you purchased it
- Lot number (if available, found on product packaging)
- Date the reaction occurred
- Description of the reaction or event
- Any medical treatment received
- Photos of the reaction (if applicable)
