How to Report

If you believe you have experienced an adverse event related to a Hizashi product, please contact us with the following information:

  • Your name and contact information
  • The product name and where you purchased it
  • Lot number (if available, found on product packaging)
  • Date the reaction occurred
  • Description of the reaction or event
  • Any medical treatment received
  • Photos of the reaction (if applicable)
